Following are the requirements needed for application depending on your type of business.

Corporation

1.	Duly accomplished WQAP application form
2.	SEC Registration
3.	Business / Mayor’s Permit
4.	Company Profile
5.	DOH Certificate of Product Registration
6.	Secretary’s Cert. - authorized representative
7.	Picture of authorized representative; (4 copies, passport size)
8.	Annual Membership Fee - P5,000.00

Non-Corporate

1.	Duly accomplished WQAP application form
2.	DTI / SEC Registration
3.	Business / Mayor’s Permit
4.	Company Profile
5.	DOH Certificate of Product Registration
6.	Seminars regarding Water Purification Treatment
7.	Picture (4 copies, passport size)
8.	Annual Membership Fee - P2,500.00

Water Refilling Stations

1.	Duly accomplished WQAP application form
2.	DTI / SEC Registration
3.	Permits- Business / Mayor’s, Operational, & Sanitary
4.	Latest laboratory tests- bacteriological, physical-chemical, & heterotrophic plate count
5.	Seminars regarding Water Purification Treatment or UP 40-Hrs Basic Cert. Course Certificate
6.	DOH Certificate of Product Registration of equipments installed
7.	Picture of equipment installed
8.	Sample of sticker / label
9.	4 copies ID picture, passport size
10.	Annual Membership Fee - P1,500.00
